Red Oak Chiropractic & Therapy offers our patient form(s) online so they can be completed it in the convenience of your own home or office.

• Fax us your printed and completed form(s) or bring it with you to your appointment.

New Patient Health History Form - Required

This lets us know the history and current state of your health. What questions, concerns, goals, regarding wellness can we help you with? Let us know!

Our front office staff will be happy to email you a link to our patient portal where you may complete your paperwork online PRIOR to your appointment with us.  They will also email you a temporary password so that you may access this portal.

This information must be completed prior to your appointment so that we may honor your appointment time with us.  If you are unable to complete the paperwork prior to your appointment it may be necessary for us to reschedule your new patient appointment.  Should you have issues accessing the portal please call our office so that we may assist you.  If you are unable to complete this outside of our office, please arrive at least 45 minutes prior to your scheduled appointment time so that you may complete the information in our office.

Member Wellness Registration Form - Optional

This form can be filled out to register for access to the member wellness section of our website. You can also sign up for our monthly newsletter to keep up on current health issues and news and events in our office. You can print it out and bring it in to our office or Click Here to register online! The online newsletter sign-up is also on the right.
